If one or both the parents are out of Kuwait, legal guardian has to submit an official Authorization letter from the parents stamped from Kuwait Embassy where the parent reside along with parent’s Passport copies.

If a single parent bears the custody of a child, custody document copy and translation in English should be provided.

If the custody is with the grandparent, then form C “Authorization minors” must
be filled in and the custody copy and translated in English.

Marriage Certificate and child’s Birth Certificate copy of the original and translated in English.

If one of the parents is dead, legal guardian has to provide the death certificate
